See what they respond and act accordingly. As a backup, keep a new corrected form ready, in case they ask for it. Since the form is for the Sponsor it is more useful when the file is here in Canada.
If they do not respond, then send the email to NDVO once your application reaches there.

I think you shouldn't panic just yet as 2 weeks is Avg time for Ottawa VO to issue Landing Visa. I would suggest to wait for another week and if you don't hear by end of next week, you can inquire then. Best of luck. I don't think there is anything to worry now at this stage. The Answer is NO, as the provincial Nominee Program doesn't fall under Family Sponsorship category. It will now fall under Express entry. That is not correct. Many Provinces have (or at least had) programs to sponsor relatives. Those programs for sure do not fall under Federal Family Sponsorship category but they are still sponsorship programs. The question is 'Were you yourself sponsored to come to Canada?'. It does not say whether under Federal or Provincial.
