You are suppose to send out the receipt which you get from the doctors office. the actual reports will be sent directly by the doctors office.

There are no forms for re-medical. If instructed by NDVO, make an appointment and get your medicals done again. If NDVO haven't instructed you i would wait since they might ask you to apply for PPR on the priority basis that your medicals might be expiring soon. has happened with others very recently.

Member of Parliament in Canada. They might call CIC on your behalf and get some info for you. more chances of this happening is when you go to their office and meet them in person. On a call, you wont get any benefit out of it
