It's been 5 days since DM, but no PPR request yet....surprising... and the wait goes on and on and on :)

If you are ordering notes you have to fill out the consent form.Fill out section A with principal applicant info and in Section C put the info of person you authorize to receive the note.When you order notes there is a option to upload the form.And also $5 fees.

Printing on paper or from photographer, either of them is fine. It's good to write on the photos (either on the back or as a caption) about who is in the photo, what is the ceremony, place, etc. Don't worry too much about the quantity of the photos. Focus on highlighting all the aspects of a traditional marriage and your honeymoon. Make sure you cover your family, friends, your spouse's family and friends, all rituals you had for your wedding (including pre-wedding and post wedding if you had), all landmarks you visited during your honeymoon. I sent around 40 photos each from my wedding and honeymoon.
Make sure you add a cover letter mentioning your UCI, application no. and passport no. Also, describe in detail what extra evidence you are sending.

Btw do you know if the engagement pics which you sent to CPC-M is added to your application?

Hi guys,

Good news for me after a really long wait. She got re-medical request today 14-04-2016.

My application is as follows:

We received application for permanent residence on January 26, 2015.
We started processing application on April 14, 2015.
Medical results have been received.

The line for re-medical is not added, but we received on the above mentioned date.

The confusion is whether the consent form should be signed by only the PA or by PA and sponsor both. Nabaaf posted that both should sign and then I posted a thread that mentions that only PA should sign. Can you clarify on that or someone else who has ordered notes before.

Only the principal applicant should fill Section A and Section C as and sign the form.This is what i do and i had no problem with it and done it twice now.

The info provided by me is followed by most of the senior members n they were successfull in receiving their notes.....members those who hv already applied fr it will gv us better picture.....u may even check 2014 and 2015 spreadsheet tab fr the procedure of applying notes ...

Only request letter stating reason for gcms notes along with pp copy from primary applicant works if they are in canada...if primary applicant not physically present in canda then consent forms required
