Yes, you have to sign the consent form. After signing it you can scan and send it.

anyone in this forum who applied for notes in the last 10 days? has something changed in options?

I applied yesterday. Didn't notice any changes in options or questions. I followed the video tutorial. But i haven't received their confirmation email, however I got the receipt printed at the end of application. I'll give it another day or two & then contact them or my bank. They haven't charged my credit c. either.

Im guessing u r the applicant right?

If consent form, it should be:
Your Full Name: Sponsored spouse (Principle Applicant ie. you I suppose)
Your Spouse Full Name: Your spouse (Person who is sponsoring you)
Your designated Individual's Name: If your spouse is applying GCMS notes on ur behalf, then your spouse's name. If a lawyer/representative is applying for notes on your behalf then their name.

Guys just saw on a other thread that August 2013 applicants from sydney are getting their visas already!!

Sydney also was on strike, sydney also has a 11 month timeline

Whats wrong with NDVO if they are still clearing 2012 and 1st quarter 2013 applicants!!

This just seems wrong!
