Yes, one photo. They attach it to the medical form.

Doctor Mascarenhas actually has eMedical. Still, the receptionist asked me to to get a photo. One guy who didn't get the photo was asked to go and get it before he could take the test.

To be sure, call the doctor and check.

Weird, as they'd have to scan that photo in order to upload it with the results...

Yes, they attached the photo and scanned it. They gave me a photocopy of the form as confirmation.

Do I need to send this confirmation to cpp-ottawa ? No reply from Ottawa so far about my wife's medical - she already completed an exam for trv couple of months back, I wanted to check if they can reuse that result.

Weird, as they'd have to scan that photo in order to upload it with the results...

Medical Request Form was in my spam folder, was sent Oct. 15. When I do my labtest and xrays, does it need to be the accredited ones for CIC like the panel physicians? I can't find the phone numbers for these laboratories?

Do not do x ray and lab tests separately on your own. Just go to one of the doctors in cic panel (check cic website for the list). The doctor will send you to one of the lab and x ray place. Lab and x ray results are directly sent to the doctor.

I had been to Dr. Lyndon on 16th Oct..............

Doc Lyndon himself actually clicked a picture of me with a small pocket digi-cam.

I dont know if my medicals have reached CIC or not. I have ordered GCMS notes............will update on 1st Dec.....

Hello,

I tried calling Dr Lyndon office 4-5 times but no response or returned call. I will go with Etobicoke Dr on Nov 15th.

On the RPRF :
If I am sending the form in email, so I still need to send the hardcopy?
The unit they refer is in Inches or in cm. I know it's very small thing, just to avoid any confusion! How did you guys mention ?
