Never got my PR Card

Hello.

I became a Permanent Resident December 2008, but never sent my address and never got my PR Card. (I know it was dumb)

Obviously I have an address now. But I'm not sure how to proceed.

Can someone tell me if I need to submit an application? Or can I still give my address and have it mailed like I was supposed to in the first place.
