Need URGENT help on filling Schedule 4A Provincial Nominees

Folks, I have received the same request to send Form Schedule 4 & 4A, I have been nominated for International Skilled Worker, SK- Express Entry.

Since I am not applying as business nominee, I am planing to send a self declaration stating that I am applying under Skilled worker and not Business nominee. and fill the form with basic details and upload it like many have suggested.

Please let me know if anyone has gone past this stage after submitting a letter and received PR.

ALL Provincial Nominees have to fill this form (irrespective of whether the applicant is self-employed or employed).

Fill it, print it, sign it and upload it to the respective placeholder.

Schedule 4A - http://www.cic.gc.ca/english/pdf/kits/forms/imm0008_4Ae.pdf

ONLY self-employed people need to fill this.

For those to whom this form is NOT applicable, just write a simple letter and tell them Schedule 4A is not applicable to you and upload the letter instead of uploading S4A.

To conclude, self employed people have to fill, sign and submit both forms whilst others have to only submit the former.

Form 4 is to be submitted if you have been nominated under economic immigration -skilled worker category. Form 4A is to be filled by persons nominated under business/investor/enterpreneur category.

Schedule 4A is for Business Nominee, so it is not suitable for me.

Hi rajkamalmohanram.

I did the same way as you suggested. My question is that even after uploading in the two enable fields in express entry profile, the indication of 'Your action is required' has not changed. I received request to upload these documents on Jan 14, 2016 with one week deadline and I uploaded them on Jan 15, 2016 but still no change in the status and/or indication.

I just want to make sure if everything is normal. Do I also need to press 'NEXT' at the right bottom of the documents-upload-section? I didn't do that bcz I have already paid the fees. Pressing 'NEXT' will bring me to the next page i.e. payment section.

Please guide.

Q 1. Which Provincial Government Has Nominated You?

I got nominated under OINP. I am confused whether to mention

a) Ontario
Or, b) Ontario Immigrant Nominee Program (OINP) .

Q 2. After filling the form, Do i need to take print, then Sign the document, scan and upload or
Just enter my name in the signature box and upload.

Please experts need your valuable advise.
