Only need to be certified copies of documents that are not in english(or french). For example: if your birth certificate is in any language that is not french or english, you need to send translation with an affidavit of the translator and a certified copy of the original birth certificate

Hi,

I came to Canada with my wife. Initially her status was Visitor but after few months I applied for her work permit. Now when I am filling IMM0008ENU_2D (Generic application form), under Dependent’s personal details section, point 13, it asks for current country of residence, status and from & to dates. “To date” would be the work permit’s end date, but what shall I fill in “From” date. Shall I give when she first came to country or the date when she got her work permit?

My second question is related to me i.e. Applicant’s Contact info. My work permit is ending and so I am going back to my home country in two months. Now confusion is what address I shall provide in Current mailing address and Residential address. I am thinking of giving my home country’s address under Current mailing address and current address (i.e. Canada’s address) under Residential address.

Please advice.

i guess you are talking about form imm5562 "your travels", if so...
you need to include all trips you have taken outside your country of origin or of residence in the last ten years (or since your 18th b-day if it was less than 10 years ago) if you have not travels you just fill with "did not travel"

you are right because you are going to receive any communication they send you to your mailing address (or e mail if you provide one) so is good to put your home contry's, if you are going back there; and your current address will be the current address at the time you submit your application, you must contact cic if you want to change your mailing address
