how do I get my hands on this infamous form ... a blank one?
I know everything else I got to do once I get it , but how to get a blank? That is the question.

I live in Montreal and I don't really want to travel to the Philippine embassy in Ottawa.
Does anyone here have any experience with getting a blank of this form?

try to phone the Philippine Consulate office in Montreal and ask Them first if there is available NBI Form No. 5...

Hello!

I got mine by personally visiting the Phil Consulate General of New York. However, they also have an option of requesting the consulate to mail you the NBI form. Maybe the office at Montreal can also mail you the form?

Yeah I did phone the Montreal office ... they're only a satellite station for the embassy in Ottawa ... doing a courier service from Montreal to Ottawa
They told me to go to Ottawa. When I phoned there, I realize now that the person there thought I already had an NBI form the way he answered.
I'll have to phone again on Monday but I'm mailing them a letter requesting the form today anyway.
I'm running out of time with all this bureaucracy
can you believe that the Philippines won't even accept an official GRC C 216-C (RCMP) fingerprint form?!
that was never mentioned on the CIC instructions , I wasted my time and money doing that.
I have EVERYTHING to do a PR application for my partner except these stupid police clearances.
very, very frustrating
 
Oh yeah! Their instructions on the CIC website weren't really that clear.

I'm not sure if you know this already, but I'm telling you anyway to be sure.. After you get your NBI form, have the details completed and have your fingerprints taken, you still need to go to the consulate general to have your NBI form authenticated. And I paid $25 for that! Had to come back the next day to pick it up.

So when you call their office, ask if the satellite office can authenticate the NBI form for you to save you the trouble of travelling far.

what I did was include an X-Press Post self addressed envelope in the envelope I'm mailing to Ottawa including a bank draft for 28.75 ... just to get that NBI form mailed to me nice and pronto
then once the fingerprints are done I'll send the form back to them with another X-Press post self addressed envelope and another bank draft for 28.75 ... just to make sure they're nice and happy and get my "consulated" form back to me ASAP

You can write the Vancouver philippine embassy with a SASE (self addressed stamped envelope) and they will send it in about 3 weeks.
