@ Keypad: Not sure about British Columbia then..You may have to talk to someone there personally I guess...here as I said (in Ontario) the format was <LastName>, <FirstName>.

Good news is - I got my PR card updated today with split name. So everything is done. So just to re-iterate for everyone - here is the process for splitting the name:

1) Get a name change certificate from your province you are residing. (Since I live in Toronto - I applied the same in Service Ontario through mail). Name Change Certificate came after 8 weeks.

2) Once I got the certificate - took it to local Service Ontario office. They updated my name in Driving License, Health card.

3) Took the certificate to local Service Canada office. They updated my name in SIN Card.

3) Mailed the copy of the name change certificate to CIC (Immigration). Use the application of PR renewal

4) Received the PR card with updated name from CIC office.

5) Only thing which will not be updated is COPR because I changed my name after becoming a permanent resident. (FYI-COPR name change is only done if there was a mistake done by immigration officials.)

I have been following this thread since a long time coz I have renewed my parent's passport with split name. Earlier they didn't have surname on passport which was causing them difficulties while boarding at airports. So, now their passports have given name and surname both.

I have to get their PR card updated with split name. For that Do I need legal name change document? I read somebody's post who got his name on PR card updated without using a legal document. Is it possible to get name change without any legal document? If no, then I have a few questions below:

Q: 1 Can you please explain which form did you fill for name change to get the document from Service Ontario? Is it 11155E form (Application to change an adult name with $137 fees)? Because they have not actually changed their name. It is just a Split of given name into first and last name. Even then do I need to fill this long form of 17 pages and provide all info?

OR

Is there any other form just for split name cases? I went to local service ontario centre to enquire about this. They know nothing.

Q: 2 For PR card which application form did you fill? Is it IMM5444 (Application for a PR Card)?

I have the same issue and a recent pr. Issue is with cra not being able to process my registration due to missing given name. I saw the process you followed,can you pls tell how to contqct immigration? Is there an email or helpline number?

Also an additional question..i moved on pr in april 2018. And by going through the above steps..i wont be able to apply for correction of name until i have spent a year in ontario?
How do i fix my name as its affecting my applications with CRA on sin and they point me to service canada and service canada say they cannot change and can only reflect whats in copr.

Did you stay for more than a year to apply for service ontario name change? Is there another way to get legal bame change?

Hey Manu.. Thank you for your update. I too have the same problem but will the surname being blank cause a problem for citizenship.
What application form should we select inorder to send to CIC? Did you do this online or via mail.

Hi,
I had the same problem. My passport had only given name, because of which my COPR and PR card had captured everything under last name.

I have got everything rectified now, without getting any affidavit or legal name change procedure or name change request through any provincial offices.


1 - Submit a letter to BLS (along with passport application form to get a new passport with first name and last name) explaining the error and why you could not get it rectified earlier. (In my case the PR application was under process). - Got my new passport in 20 days

2 - Once you receive the updated passport - Submit similar letter to CIC explaining the error, along with other documents - Application form (Do not tick legal name change as you are not legally changing your name) + Cancelled page of old passport showing the new passport number and copy of the new passport showing old passport number, ensure you mention this also in the letter) + copy of the letter you submitted to BLS + any other documents. - Got my New PR card in 72 Days, Please note COPR will not change, only PR card will issued with a new name.

3 - Got my SIN, Driver Licence and Health Card updated.

Its an easy process if your letter is crisp and explains things clearly. Let me know if you have any doubts.

I am currently starting the process of the name change for my wife.

She has received her Indian Passport last week, with "Split Name".

We have moved to Ontario in May 2018. It has been less than a year.

Can you guide us with the "Step 1" - Get a name change certificate from your province you are residing
since i understand that you were from Ontario as well.

Did you not have to go to the lawyer ? as you mentioned in your previous post

:confused:Hello Everyone,

I read this whole thread, I have the same issue with name
( Abc Singh) written together in my Indian passport in First name field and last name is Blank

and my Canadian Immigration documents ( PR card, landing paper ) has it in Last name field and First name blank, but apparently my Driving license and heath card has last name, first name.

I have given my citizenship test last week and going for oath ceremony next week. Do I follow the same procedure for name split ( Apply for legal name change in province( Sask) then forward it to IRCC to get new cert of citizenship and then apply for Canadian passport?

How BLS will issue me OCI card in new name? Do i have to make name change in Indian passport as well, even though I will be becoming Canadian citizen very soon.

anybody know if Canadian Passport center can issue passport with split name without any documents in support or just my drivers license & health card?

I have sent a copy of my new passport, a letter requesting a change of PR card and or renewal form to get the last name on my PR card. When I checked my Ecas after receiving AOR, I have to put my full name on the last name to check the status. Is this common?

I am just sharing my experience:

I sent my PR card renewal application with new passport that has first name and last name clearly separated. But they asked me to submit a legal name change certificate. I submitted the fingerprint for RCMP check (mandatory in Alberta for legal name change). After I received the fingerprint clearance in mail, I submitted the name change application through registry. It took about two weeks to receive the certificate, after I received the name change certificate I changed my drivers license and provincial health card, sent the legal name change certificate to cic. I received my new PR card within two weeks after I submitted the legal name change.certificate to them

Hello all,

Finally, I got my name split done on each of my documents. Whatever problems I was facing due to missing last name are now resolved. I am describing my journey here for all those who are still deciding or want to know the process.

**Note: I did this in BC but most of the things will be same for other provinces too except step 1.**

1. First of all, I needed to apply for a legal name change process. This was done with Vital Statistics. It took almost 5-7 weeks to get my legal name change certificate.
2. Next, I went to my nearest Service Canada location to get my SIN number updated. This process took half an hour and issued me updated SIN letter on the spot.
3. Then I went to ICBC to get my driver's licence and BC Services Card updated. They also took close to half an hour (including wait time) and updated my licence as well as services card in their records. However, I received my new licence and services card in a month or so. Other than this, I had to sent a copy of my name change certificate to HIBC and requested them to update their records.
4. Along with step 3, I sent my application for PR card replacement with amended name. They took close to 2 months.
5. Then I applied for my home country passport renewal with proper first name and last name. Surprisingly, they took only a week to issue me my new passport.

This entire process took almost seven months and finally sorts out every issue related to missing last name in my documents.

Thanks to everyone who guided me through out the process.

Sorry for the late reply. I was not checking this thread on a regular basis. You need to fill the form, get the verification of your fingerprints, attach any necessary documents and mail them to Vital Statistics. Adding actual surname or getting your full name split is entirely your choice. You can do whatever you feel better with. In our case, we just got them split as we never use surnames. Once you receive your name change certificate, you will need to apply for PR Card renewal form and mail that along with necessary documents to CIC.

Thank you so much for your reply.I was filling the form and the name in the foundation id is considered as a surname ,so should I leave the given name portion empty or should i fill the name as xxxx yyyy in surname and given name too.Also,Do the indian passport office ask for any kind of advertisements in newspapers as i m just going to split my name.Finally,can I apply for reissue of passport before having a pr card?
