I would like to mention my experience here. I tried my best to get a letter on the letterhead (for my OINP application). I contacted my manager first and requested him to provide a letter (I showed him the format). My manager refused to give me the letter (because he said he is not authorized to provide such a letter). I then e-mailed the HR and asked them to provide such a letter on the letterhead! I pursued them for 3 full weeks but to no avail.
I got the letter from the HR in the standard format of the company (It mentions only my name, date of joining, designation and employee ID). Then, I got the letter from a senior colleague in the required format in a plain paper. I suggest all applicants to get this letter notarized. To sum up, I submitted the following documents to OINP -
1) Letter of explanation for reference letter
2) Copy of the Standard format of the company’s letter
3) Reference letter(with all necessary details) by a senior colleague
4) Copy of the senior colleague's company ID card
5) Copy of Email – 1 from the HR
6) Copy of Email – 2 from HR
7) Copy of the company’s HR policy
8 ) Copy of pay scale letter for 2015
9) Copy of pay scale letter for 2014
10) Copy of the pay slips for the past 6 months
11) Copy of appointment letter [joining letter]
12) Copy of initial job offer letter
Luckily, my application was accepted by OINP and I got my AOR. As you can see, I cannot provide any more documentation to prove my work - I had submitted ALL the documents that I had.
So, if your letter is not on the letterhead, you can just go ahead and submit a letter from your supervisor/colleague & get it notarized. Please note that the name, phone number, official E-mail ID and the designation of the person giving the reference MUST be mentioned on the letter. If you provide comprehensive documentation, it is likely that OINP/CIC will be satisfied and proceed with your application.

I understand there is no such standard template and content varies significantly with the one provided in this post. As am in the process of documentation, I didn't want to screw this. Hello aju, Hope you got what you had asked. Would you mind sharing me the content of the affidavit used. It would be of much help to me as am in the process of collecting documents.Hi, Sorry to trouble with this doubt..
I got ref letter from my previous company, TCS in their default format without any info like hours, designation etc and it has only 5 points about my job roles. Also it doesn't have the contact of my manager who approved the letter nor his designation. Also, he is not ready to give a notarized affidavit as well - he is scared to do so. But may be he will give a copy of his TCS ID card.
I have 2 colleagues whom I think can help me. One was my senior and the next one was in my same role.
So my questions:
I am going to take notarized white paper affidavit from the said ex-colleagues. They will mention exactly like "This is to confirm that the xx had been working with TCS in designation Sr. Process Associate from xx to xx as a Permanent Employee and his working hours were 45 hours a week. He was working as a Claims examiner for the Claims wing of leading Insurance Company. As per TCS norms, more details of the client cannot be revealed. His annual salary was xxxx and his duties are as mentioned in the Reference letter issued by TCS and the same are re-quoted below...... ".
And I will include their ID card. But I cannot prove their designations other than by their signature on the white paper.
1. There are only 5 roles mentioned in the official ref. letter. I think this covers many points in NOC site. But I read in forum that 8-10 points are required. What I will do?
2. In the affidavits/ ref letter that I am going to take from my colleagues will restate the same points of my official letter. Or should I add some more points? May be this will confuse the CIC persons. Please opine.
3. And more over, is this the correct step that I should be taking?
Please give your opinions and inputs.

Thank you for your information. Understood, you mentioned your designation in the affidavit. Where did you mention your colleague's designation?My reference letter had everything IRCC wanted; but it was not on letterhead.
Name, designation, tenure of employment, annual compensation, roles and responsibilities, contact information of the person who signed the letter.
Am no expert, but you are moving in a right track. Take a print of the email and get it notarized. I believe, your manager's signature will be already available in the said email along with contacts.Hi Experts, Need advice.. I am working in India in a US based MNC. I requested HR to issue me reference letter with duties & responsibilities but they refused. Instead they issued me a letter with in standard format without duties & responsibilities. I requested my reporting manger for the similar letter but he told me to talk to HR. I tried again and requested my manager. This time he sent me an email mentioning my duties and responsibilities in detail. I am thinking taking print out of email and get notarized and submit along with LOE to IRCC. I do not know whether this will work. Kindly help. My deadline for ITA submission ends on 3rd Aug. Thanks.
There's a notary template for affidavit that needs to be put at the bottom of the letter after all his declaration about your work experience an job duties. You could Google and get that, and put in your letter.Hi Seniors,
I am working in US and cannot ask my employer to issue me a reference letter as that would jeoperdize my job.
Can I get my job responsibilities typed on a plain A4 size paper from my colleague and get it notarized ?
If so, can somebody please tell me how can I get this document notarized (or have an affidavit created) in US. Actually I don't know the legal process here, so somebody who has similar US experience can guide me. Thanks for your help.

I googled the template you mentioned; just to confirm is like following:There's a notary template for affidavit that needs to be put at the bottom of the letter after all his declaration about your work experience an job duties. You could Google and get that, and put in your letter.
Your colleague needs to print that letter, and take it to nearest UPS store that offers notary services, and sign it before them. Once he signs notary will put their stamp and sign on that content that you pasted for them at the bottom of the letter.
If he's a bank of America customer, he can go there for notary too.
