My son name is missprinted on his medical form

Today, I have received IA. But they have printed my son name on his medical form in wrong order (they put the first name as sir name, sir name as first name). On his passport, his first name is printed at the place for the first name, and last name is printed at the place of last name. Moreover they did not stick his picture on the form nor they stamped it. Is that ok? Should I go for medical? Or Should I contact cic office first.

try to email the cic so that the correct their files for corp to be correct when they issue it. i dont think there is a problem. the dr wll enter corrections as on ppt and they will be sent with names on ppt. let the doc know be4 u oay for med

I contacted to the Buffalo office via email. They replied me within two days. They said that the corrected form had been printed and sent to me.
