Dear All,

I am helping my friends for the documents to CIC,mine will be prepared after getting LOA, and have many questions:

in the form of IMM5669E,no.12 Address:"list all address where you have lived since your 18th birthday or the past 10 years."
How about the below situation:

2004-2008 University lived in the campus(City C) went back home twice a month(City B)
2008-2010 Working lived in a rental apartment(City D) weeked at home(City B)
2010-Present Working lived in a rental apartment (City E) (got married) went back spouse's home weekend (City F)

I know it is a little complicated, but there might be some people with the same situation, study and work in the different city from home,so how to make the list in the form? could you please give me your suggestion?thanks very much

2001-2010 City B
2001-2004 City A
2004-2008 City C
2008-2010 City D
2010-Present City E
2010-Present City F

At the last row of the table, write "Refer to attached explanation regarding overlapping stays in mentioned addresses". Make an additional sheet for the explanation.
Write the following in the heading of the additional sheet...

Dear ad123164,
thanks for your reply, is it how you did in your form?

Yes, I'm an OFW living in Jeddah with my family. We go home twice a year. My family spends cumulatively about 3 months in Manila and I spend about 1.5 months in a year. As such, I have to put both addresses with the same period.

Hi!

RE : Online Application.

Do I have to put the middle name? or just the first name? I am starting to fill up.

Thanks in advance.

Additional Dependants/Declaration [IMM 0008DEP] - will not fill up since my spouse is my only dependent
Schedule A – Background/Declaration [IMM 5669] - Principal and Dependent Applicant
Additional Family Information [IMM 5406] - Principal and Dependent Applicant
Schedule 4: Economic Classes: Provincial Nominees [IMM 0008 SCHEDULE 4] - Principal
Schedule 4A: Economic Classes: Provincial Nominees – Business Nominees - will not fill up since im not applying as Business Nominee
Supplementary Information - Your travels [IMM 5562] -Principal Applicant
Separation Declaration for Minors Travelling to Canada [IMM 5604] - will not fill up

hi binx, We are on the same timeline and my sponsor was interviewed last April 30, I am patiently waiting for the letter ...let me know what other options can I do other than waiting .... :-[

hi rh3ia , i noticed in your other response that you have called manitoba to validate if they have sent you the LOA already, can you give me an information on how you made a followup ?

Hi Ramcal, maybe you can start applying for your NBI clearance... they gave me 15 working days before i can pick up mine.... so when your LOA arrives, ready ka na to submit to CIC.... think positive LOA na yan :)
