I was able to get my NBI clearance in advance.. when my sponsor was interviewed me and my husband applied nbi clearance online right away..hehe its much faster and convenient using their online facility but i think the area is limited..

by the way i noticed the forms have diff years, some are 2004, 2008, 2012, 2013 etc. as long as we download the form on the their site regardless of year it's ok? hehe

do you mind, may i know your civil status?

I am still single, so if ever i will go there alone. :(

regarding the year in the form, just download the latest version - the version in the link I have shared earlier. there are some forms that were revised by them every now and then. better to get the latest version always. and there is no better place to get it but at their official site. it is too risky if you will get the form from other sites for it may not be the updated one. ;)

1. in the generic form (IMM0008) total no. of years of formal education, does it mean.. elem+hs+college? :)
2. for my spouse (dependent) under question 16. c provide the name of current spouse, family name and given name(s) will i write my maiden name or my married name?

I am helping my friends for the documents to CIC,mine will be prepared after getting LOA, and have many questions:

in the form of IMM5669E,no.12 Address:"list all address where you have lived since your 18th birthday or the past 10 years."
How about the below situation:

2001-2004 high school lived in the campus from Monday to Friday (city A) weekend at home (City B)
2004-2008 University lived in the campus(City C) went back home twice a month(City B)
2008-2010 Working lived in a rental apartment(City D) weeked at home(City B)
2010-Present Working lived in a rental apartment (City E) (got married) went back spouse's home weekend (City F)

I know it is a little complicated,but there might be some people with the same situation, study and work in the different city from home,so how to make the list in the form? could you please give me your suggestion?thanks very much

1. Yes, that is correct (6+4+?)
2. It should be your family (married) name

I've just written "MANILA" in my application.
