No form for rprf payment. Just prepare a bank draft/mc for the amount of rprf you need to pay, check link below for details.

I personally drop my payment in their drop box at basement 4 of rcbc tower because its very near on my workplace.

You can choose to send it thru courier any courier will do.

Our medical is schedule on sept 2 at IOM in Makati, since its more accessible to us and its also by appointment. I've read positive feedback about IOM so we choose them.

Haven't experienced anything not good I arrived at about past 6am there,
have my docs checked by the guard before entering the hopital's premise.
I proceeded to Canada Reception, have my photo taken there after submitting all
the requirements, then I was asked to pay the fee. After the payment, the tests followed.
X-ray
blood and urine
vital signs/height/weight/eyes
physical exam

Finished all the tests by around 10:30 (they have informed me that lab tests are all okay) then I was asked to come back around 1:30 for the result of X-ray. Then came 1:30, already have my receipt with me.

Just sharing as well..

If you're going to have your ME at SLEC, I suggest you go there on a Friday. It's not as crowded as other days.

I also had a pleasant experience at SLEC. It's first come, first serve though and come with extra cash because they require additional tests that are not included in the standard fee but that would depend on the medical history that you will provide.
