Can someone help me get a good format in which to write a letter of explanation to put with my application explaining my missing clearance?

Then it won't be considered missing. It would only be considered missing if it was required to be included upfront. Send your app as is and CIC will send you the request letter later on.

For countries that require an official letter to issue the PCC, the existing procedure is to send the app and wait for the letter to apply.

Again, without knowing the country, I cannot answer. You have to read the country-specific instructions that CIC provides.

When should I start applying for PCC? Should I wait for ITA or shall I start as soon as I submit my profile?

It's in my timeline/signature so about 3 weeks from AOR.
