Missed out IMM 5406

I'm kicking myself here as I have just realised that I didn't send in my IMM5406 - Additional Family Information. I have no idea how I skipped it / forgot to include in. I guess the stress got on top of me? Who knows. Anyway, I don't have my AOR yet as it only arrived in Mississauga last week. Anyone have any ideas on how I can rectify it? Or should I just wait for the CIC to probably send my whole application back.

I would ring the CIC but I've not got time for the merry-go-round antics of the call centre.

The call centre wouldn't be able to help you anyway, they'll just say it hasn't been opened yet. They may send it back (which is my inclination of what will happen), OR they may just let you know 'this or that' is missing please send . . . etc. etc. I wouldn't suggest trying to supplement your application with another envelope, someone else tried that here and it ALL got sent back.

I missed out the IMM 5406 as well, but after about 3 weeks CIC sent me an email asking me to send it, which I did immediately. The only repercussion was that my application was slowed down by about a month: I suspect it went to the back of the queue. I would definitely try sending it off now: what do you have to lose? There are only three possibilities: either they will join it up with your application and deal with the application as a whole within the normal 1 month timescale, or they will fail to do that and as in my case ask you to supply the missing document or they will fail to join it up with your application and return the application to you. The email I received from Mississauga asked me to send the missing document to Application Intake, CPC Mississauga/CTD Mississauga, PO Box 6100 Station A/CP 6100 Station A, Mississauga ON L5A 4HA. Why not try that address? You will not have an UCI or other reference number so give as much detail as you can: full names and addresses and dates of birth of yourself and the sponsor, date of delivery of your application at Mississauga if you have that, and the name of the person who signed for the package containing the application if you have it (I sent my application by Fedex which supplied that detail). Also keep a photocopy of the completed IMM 5406 form: you may need that!

Hey... i dont think sending it now will help..they wont be able to identify and attach it to your file as you dont have a UCI or Application number yet.

Best is to wait for them to ask for it..this could also mean them sending your file back! :/ So damage already done....

Sorry about this...i also lost out on a month when my application came back for being incomplete as well.

You do have a UCI on your IEC visa. Its the same number they will use for this application. So you can put it on and it will help them match it up

Exactly. I think people on here just assume it's your first dealing with the CIC when for us it's our 4th. If it comes back it comes back. If it doesn't it doesn't. Probably will just wait it out since they'll probably look at my file soon anyway. Infact since I've filled 3 times previous with noting my family members, common sense would say that they can just pick it up. But I know they won't.

Imaging the thousand applications that CIC gets everyday....it will be a miracle if they match ur form to your file without any delay even if it your 3rd-4th dealing with them..

I got myself into the same situation as yours, i wonder if you can give me some info on how your case went.
Did CIO request you to send this missing IMM 5406 form so they could complete check for the whole application? or did they return your application back and you submitted it again with the missed form?
I feel so down right now for missing this form.
I just sent my package yesterday and called the Post office to see if they could return it to me, but looked like they can't
