I have got my passport back with visa on Jan 15, 2013.
Thanks everybody for all the help that I got on this forum.

Recieved file : april/2012
Start processing:june/2012
Medical request:August/2012
Medical done: 5-sep-2012
Medical in ECASE:15-10-2012

And Till now i didn't get any PPR request.
Yesterday i sent email to JMR to ask for any file update and when i can get PPR request,
I was shocked with his reply today as follows:

"Background checks are outstanding and will take many months to complete. We will advise further when in a position to proceed"
I need your advise guys...what does that means?

I see that they are issuing visas based on passports received at CHC (exceptions always exist). They reached till November 30, 2012. December Passport submissions will start receiving their visas now.
