Tomorrow I'll go another physician....

Yesterday I received gcms notes which generated before 21 days of my MR mailed. Mentioned there " This file is ready & instruct for 4 MR. Rprf, ppr etc......

do u know the UCI of all memebers including you. if yes then with the letter u have and UCI number the medical can be done by emedical center. I m saying so coz during my medical I was asked for UCI by the clinic. Missing forms also happened to me but next day I wrote to my visa officer and he sent the forms immediately. I can understand ur mental situation. i dont know what to say.

Dear Experienced Brothers, Please help me regarding submission of following documents:

MR: Will the medical centre provide me the report or submit online? Will they return all medical forms? [CIC has sent me 04 medical forms with different UCI & IME number]

Passports: CIC has instructed me to submit all PPs to CVAC [Dhaka, Chittagong or Sylhet]. Do I need to submit any other documents with Passports?

RPRF: In my RPRF, CIC has not mention my name, file number or any date of issue. It is a silly mistake I think done by CIC. Will there be any problem to make Bank Draft from SCB?

Regarding Bank Draft, PCC, all Photographs & filled in form 5669 [CIC has again asked those!!!], I need to send to SGVO using courier, right? Do I need to include the medical test result? [Or the Medical Centre will upload?].

And what else left? Experienced brothers please help me out...

Go to any of the panel physician with medical forms, passports n fees. Physician will sent medical report directly on behalf of u. Medical centre will give u money receipt for payment only

Go to Standard chartered bank with
1. Money receipt of medical diagnosis payment
2. Original Nikahnama n marriage certificate ( if married)
3. Passports
4. Taka equivalent to CAD 490/ per person ( adult only)
5. RPRF instruction sheet

Collect PCC
Take photos n follow their instruction
Fill the forms 5669E
Write ur name n file number back of the RPRF draft

Now u can submit ur passports, pcc, photos, 5669E forms, RPRF draft n appendix A form @ CVAC through VFS.

Today I went to Dr. Lina to tell her about my situation. Her assistance search with my PP no, UCI no & DOB but found nothing.....

Finally Dr. told me no problem as your documents are authentic so I'll do your medical & I'll send it according to your UCI no.....Any day u can come here with your mail documents, instruction attachment & PP with 2 copy photo (each)......

I told her better I'll wait some more days & if before last week if I didnt get those forms then I'll come to have my medical here. She said ok....

I want my forum mates comments........plz

You are right but you can avail doctor's proposal. If you avail last option, u can provide CSE copy as an attachment since doctor will send your medical reports through surface mail I think.

I have seen one person done affront medicals (medical done before request) and it was duly received by CPC-Ottawa after 3 days of medical.

If I were in your position, I would perform my medical and send everything to SGVO. In the cover letter, I would mention finding no medical form but handbook and communicated duly to them. Also would attach the page 12 of the form after filling it.

IOM (Banani) & Dr. Lina confirmed me that there is no option for affront medical for Skill Immigrant....(FSW). I must have to have medical form with IME no...

Now I have nothing in hand....
