Dear Klmy,

How are you? I hope you will fine and congrats for your Canada PR . Please advice me as per below.

We already did our medical on 08-Oct-2014 EXCEPT my wife X-RAY because at that time she was PREGNANT.

Today (18-May-2015) I went to clinic and check, me and my wife want to do full medical for both of us instead of doing her X-RAY ONLY.
Because the first time we did our medical is about 7 months (except my wife X-RAY). But the clinic say we cannot any how do our medical as they need NEW IME NUMBER from CIC in order to do this. So they say ONLY go and do my wife X-RAY. Please advice me.

My wife X-RAY will submit on coming 27-May-2015. After that we should except to received NEW FORM to fill up for NEW BORN my baby girl or any other things? How long it will take?Thanks in advance.

Regarding New born baby medical - So far I know U need to do medical for your new born baby ( Only Height & Weight Test)

Did u send New Born Baby's documents to SGVO? - Updated Additional Family Information Form, Updated Generic Form, 150 CAD Processing Fee for NBB, Passport Copy, Birth Certificate of NBB & 4 Pictures

Good day. Thanks for your response. For my baby medical, please let me know as per below.

1) For baby, under 5 years old need to go for medical?
2) If yes, CIC will send medical form with IME number like our medical form to email?
3) For medical, what kind of medical check-up, need to do? My daughter is only two months old.

Before CIC send medical FORM to my email for my daughter, I am going to submit require documents (except my daughter medical FORM as this medical form come with IME number, so I must wait this form from CIC) for my daughter on coming 12-June-2015 as per below.

1) Updated Generic Form (IMM0008ENU_2D) to add in my daughter particulars,
2) Updated Additional Family Information Form (IMM5406E) for me and my wife,
3) When pay CAD 150 for my daughter, I will use credit card. So I need to use IMM5620E?
4) Photocopy of my daughter passport ( can get on 11-June-2015)
5) Birth Certificate of my daughter (already done)
6) Passport photos 4 pcs

Any other documents I miss out? Please check for me. Thanks in advanced.

Sorry for late reply. I already email to CIC to add in new born baby to our PR application, and they update Third Line on 05-June-2015. Before CIC send Forms to my email including my baby medical, I will submit as per below. if anything need, please let me know.

Good day. Regarding about medical form for New Born Baby,

1) CIC send Medical Form for your baby to your email?
2) This form is same form like our medical form including IME number?
3) What kind of medical need to do for medical?
4) Did you do your baby medical in Bangladash? at appointed clinic? For my case, I am now in Singapore and my baby is now out of Singapore as our application fill up Singapore address, CIC will ask my baby to do in Singapore.

I along with other dependents received Visa & CoPR on 11 Dec 14 through CVAC, Dhaka. The Visas & CoPRs were issued on 25 Nov 14 by SPVO. On the other hand, my baby was born also on 25 Nov 14. I informed SPVO about my newborn on 13 Jan 15. On 29 Jan 15, SPVO e-mailed me to send documents of newborn (Photo, Copy of Birth Cert & copy of PP) along with all visas and CoPRs issued for us. I sent all these documents through VFS and those were received by SPVO on 03 Jan 15. I received the MR for newborn on 16 Jan 15.... Hope you are clear now.

When you add new born baby into PR application, CAD 150 for new born baby, can I pay by online ? or use FORM IMM5620 write down credit card number and pay?

Dear Myat,

Sorry for being late.

So far i Know u need to do medical for your new born baby (Although her age is 2 months). She needs to undergo only Weight & Height Test. So don't worry about it. As you have already raised CSE for adding your NBB, I presume u will get a separate medical form for your daughter. In my case I got separate Medical Request Form for my NBB.

Regarding IMM5620E - Can u pay additional dependent addition payment via online? I found this feature only for Express Entry Candidate. If you can't select Additional dependent-not Express Entry while paying online then i would suggest u to go for Demand Draft.
