Hi all,

I have been given a medical surveillance undertaking S2.02 and have been asked to report to a provincial/ territorial health clinic within 30 days of my admission to Canada as a permanent resident.

I am planning to reach Canada for my landing next week. And I am planning to stay there only for a week since I have to return to close out other aspects at my current residence before I move for good.

Could somebody explain what exactly am I supposed to do after landing? Whom do I have to tell about this undertaking and where is the provincial health clinic in Toronto where I am supposed to visit? Whom do I meet in the clinic and what do I tell them? Do I tell them that I am there only for a week?

Any information is highly appreciated.

Wasn't there a phone number given in the letter about your medical surveillance? Here's the sample letter for TB: http://www.cic.gc.ca/english/resources/manuals/bulletins/2011/ob340-tbh-handout-e.pdf

If you'll stay there for a week, call them again before you leave and inform them you'll be out of Canada, even if you don't know when you're coming back. I would think you'll also have to do this even if you return on short visits. What's important is that you reported.

You'll have to complete the surveillance once you settle permanently. They will mail you the testing instructions on how to do it, after you report to them that you're settling.

I just landed in Toronto to complete the landing yesterday and connected with the number you provided through the link. They have me 3 community health centre numbers. On connecting with 2 of them, they were completely unaware of the process.

Do have any other details I can get so that I can at least connect with them?

So why not go with the 1 that knew the process? Or call those numbers again, and ask for someone who does know the process. It's impossible that they are totally unequipped to deal with it.

My relative was in York Region, so they reported to the York Region health centre, not Toronto, so I can't share anything specific to Toronto.

Any update ? I am in a similar situation and want to know what's the process? I will be soft-landing in Canada soon and want to know how much time I should keep aside for this. I am planning to go back to my home country in 10 days after soft-landing.. will that be enough?

I have the same issue. I've received a Medical Surveillance Form (Code S 2.02 - Inactive Tuberculosis). I will be visiting Canada for a short landing of 8 days to complete my entry.

Once I land, I will call the Public Health Authorities and speak to a nurse about my condition. On the same day, I am required to fax form IMM0535 to the nurse.

I understand the next steps are:
(a) Public Health Department sends me a "Dear Doctor" letter with instructions for the physician
(b) I have to take an appointment with a physician to complete any tests required (generally X-Ray and sputum).

Any idea how long (a) and (b) above will take? As I said I plan to be there for only 8 days so I'm quite concerned. Do I need to visit Canada regularly to continue the surveillance?

I did manage to fulfil the requirement of medical surveillance within the first week of landing. As I was staying in Mississauga, I phoned up Peel Health Region and spoke with the TB nurse. I told her that I am only there for a week. She did my assessment over the phone and asked me to either fax or email the medical surveillance undertaking form (which you will receive from the immigration officer), so that they can set me up on the system. The nurse advised that I can print the doctor reporting form for inactive TB online (rather than waiting for it in the post or via email), which I will need to take it to the doctor.

The next step was to visit the doctor. So we did go to the multi speciality walk in clinic at Queensway in Mississauga. Since it is a walk in clinic, you don’t need an appointment. They asked me to come between 9am and 3pm to see the physician and a specialist. It costed me $198.00. Specialist referred me for the X-ray which was done the same day at the same clinic. I paid about $40 for the X-ray. The reports was available within 2 working days. Based on the report and my medical history, doctor filled up the form and everything was faxed to the peel health centre (number was available on the form). I phone the TB nurse again to confirm if they have received the form and my x-ray report from the doctor. She confirmed that they have received all and will update the immigration office.

I would recommend that you take all your medical records related to your TB treatment with yourself to the doctor. My records were well kept and it helped the doctor to see what treatment I had received.

Hello thanks for this detailed post; just going through the letter that came with my COPR in November and I see that they said if the medical Surveillance form didn’t come with my other documents, I was to contact them but you wrote that you received yours from the immigration officer. Do you think it’s fine to wait and collect it at the port of entry?. I’m landing in a few days so I’m not sure it’ll get here in time even if I contact them and they send it to me.

Since nothing was omitted, I did not contact anyone, and as I mentioned I was give the form by the immigration officer. Hello thanks for replying! My letter states the same thing but the medical surveillance form was not included with my other documents, isn’t that what they mean by “if this has been omitted, contact us”?
Well since yours wasn’t included and you were able to get one from the immigration officer then I guess it’s the same and I can rest easy.

I suppose you must have gotten an answer to this. You just need to follow the instructions of the medical surveillance I.e. calling the office within 7 or 30 days of arrival (as the case may be) and then reporting for the follow up visit with the doctor who will then decide next steps. It has no effect on your immigration status however.
