MEDICAL Surveillance Undertaking- Parents received for PR AND I AM CONFUSED AS HELL!!!!

Hi Everyone,
So, my parents received medical surveillance undertaking form from IRCC this morning and i am confused on how to fill this form because IRCC wants us to submit this form within 30 days. My parents visited us 4times before covid happened on supervisa and then we applied for their PR. This form is specifically for my dad, he recieved the same sort of form before when he visited us for the first time in 2015 and we went to one of the local public health but they didnt find anything in the records. After that, my parents visited me 3 times more before and never had any issues.

On this form on Line 8, 9, 10 and 11 are very confusing to me because they are asking about the office where the medical assemsent performed? i dont understand what should i fill on this column ? then they are asking IME no ?
Line 10: Type of entry document and serial no ? i am confused as hell and i am worried because this is like the last step and i dont want the immigration officer to reject our file on these grounds. My parents medical exam was passed. So if you can please what i should on it ???

This is form i received and below is the email i received from IRCC today. Please help and guide me through.

i have reviewed your application and documents you submitted in its support. Subsection 11(1)
of the Immigration and Refugee Protection Act provides that a foreign national must, before
entering Canada, apply to an officer for a visa or any other document required by the
Regulations. The visa or document may be issued if, following an examination, the officer is
satisfied that the foreign national is not inadmissible and meets the requirements of this Act.

I have concerns that you have not fulfilled the requirement put upon you by section 16(1) of the
Immigration and Refugee Protection Act, which states:

16(1) A person who makes an application must answer truthfully all questions put to them for
the purpose of the examination and must produce a visa and all relevant evidence and
documents that the officer reasonably requires.

Please provide documentation to confirm that you have complied with the Medical Surveillance
procedures. Please refer to the attached form as your guide to the instructions.

The above information/documents must be received in our office within 30 days from the date
of this letter. If you fail to comply with this requirement, a decision concerning your application
will be made based upon the information already submitted with this application, which may
result in the refusal of your application

Did your dad complete the medical surveillance when first came on super visa?

I guess they are asking this to make sure your dad followed the medical surveillance which was requested when he came on super visa. Its only my guess, however, wait for experts opinions.
