You are not supposed to fill out "From" section for the passport return envelope. Read your PPR carefully. I'm copying from my PPR for your convenience:

If your address is in Canada, we strongly recommend that you supply a return envelope for your passports using Canada Post's premium National Xpresspost service. Do not send us an Xpresspost - USA envelope. Taking advantage of our proximity to the Canadian border, we can make arrangements to have your passports mailed from Canada using your return envelope.
Purchase a 24 x 32 cm prepaid National Xpresspost envelope (not Xpresspost - USA ) at any Canada Post retail outlet.
Complete the mailing label showing your full mailing address in the “Deliver To” field leaving the “Return Address” field blank. Make a note of the tracking number.
Your passports will usually be returned to you within 15 business days from the day we receive them. This does not include any time it may take for the Post Office to deliver them.

Windsor, ON N9A 6P2 CANADA. Make a note of the tracking number for each envelope. Please note: If you choose to select this
service, your package will be sent from our P.O.Box in Windsor and will not originate in the USA..
