OHIP Third Party Disclosure Requests
All requests for personal information must be received in writing (includes faxes and photocopies).
The Ministry of Health and Long-Term Care maintains a record of OHIP claims (also known as a personal claims history) submitted by and paid to health care providers using a patient's health number.
Third parties such as law firms and insurance companies may request disclosure of these records with the appropriate consent.
The information in a personal claims history is used for payment purposes and should not be confused with the precise diagnosis provided by a health care provider. The provider should be contacted for information about a diagnosis or any other information that may be required.
For a third party (such as a law firm or an insurance company) to make a request for the release of health information of OHIP claims records they must provide:
The name of the person to whom the health service history relates;
The health number;
The date of birth;
The period of time for which the health service history is required (records are maintained for seven years);
A cheque payable to the Minister of Finance covering the mandatory fee of $74 and;
A signed consent from the person to whom the information relates authorizing the release of health information.
