I've noticed on a lot of posts that the docs are handing out the wrong forms when you go in for the medical. Ok, so you go in with the Appendix C form filled out and ....what is the correct form you recieve back to send to CIC?
 
The original of the IMM 1017 (Section A of the medical report) that is given to you by the DMP. It will have your photograph in the corner, and a maple leaf with a number inside it will be stamped on top. It's OK if the form refers to Appendix D instead of C.
 
Just so there's no confusion: the IMM 1017 form has several copies (it's a carbon copy thingy, the doctor writes on the top copy and it's visible on the bottom ones as well). The top layer he keeps and sends with the medical results. Copy 2 (the second layer) is what you get and send in with your application.

I printed out the checklist and took it with me to the DMP, just to make sure they wouldn't argue with me. On the checklist, it clearly says you need copy 2 of that form.
