Medical forms

Hello,

I guess it's just me but I can't seem to find the medical forms for my husband's medical exam. I thought it was in the application package and forms I had but it's not and the doc office here in Edmonton said I need to bring that form in. Can someone help me out and let me know where I can find it.

It's not just you, CIC changed a lot of their processes in 2012 and they are buried in various sections on their website and not very clear. Here is link below to some other folks on this discussion which should help to guide you.

Authorized Docs have something called UPFRONT MEDICAL EXAM with UMI #, send one copy to immigration while the doc send teh other to CIC and all they need to do is match the UMI #

So save time and get it done

With the release of the new GCMS 4.0, it should be noted that all previous versions of the IMM 1017 form as well as the Appendix C will no longer to be used. With this release, there will be two newly-revised IMM 1017 forms:

IMM 1017: Medical Report: Client Biodata and Summary (not available on our website as it will be GCMS-generated along with medical instructions)
IMM 1017B: Upfront Medical Report: Client Biodata and Summary (not available on our website as panel physicians will have these forms in their office. With the implementation of the new IMM 1017B, Appendix C will no longer be required)

NB: Some authorized physicians are still not online with the new system. We found this outs after calling them. We have to print the form and bring it. They don't have forms. So, best to call your doc. to be sure.

@Korea2Canada is correct. Many doctors are not on the new system yet, my husband had his medical done Jan. 18th 2013 in Trinidad and we had to use the old appendix C form. CIC confirmed receipt of our application and medicals in March 2013 so they are still accepting it to the best of my knowledge.

I called around to each medical office here in Edmonton. I went to the one in Heritage and it was close to $400. Here is the link for all the panel physicians http://www.cic.gc.ca/dmp-md/medical.aspx
