Hello everybody!! I'm so mixed up!!!!!!

I'm with the last details, sending soon the application!

However, now I suddenly have a crisis with the medical exams forms. I'm applying OUTLAND (in Buenos Aires, Argentina) and I made my exams here in Quebec City. I've been with the Appendix C, but they kept it to send it to Mississauga with the rest of the examination. They are completed and passed. But I only have one paper from the clinic that states that I've done my medical exams for immigration, signed and stamped.

Do I put that on my application?
I don't have any other form. What about that IMM 1017 copy 2 that the checklist talks about? What do we (people applying in Buenos Aires) need to add regarding medical exams?

HELP PLEASE!! I'm so nervous about that :o
 
The paper from the clinic, signed, stamped and dated, IS supposed to be the IMM 1017 Copy 2. That is what you attach as proof of medical.

Thank you! Even if it doesn't say "IMM 1017 Copy 2" at all, and is like a preset 'letter' from the clinic? It's signed, dated and stamped (with a red maple leaf with a number inside, I guess the DMP number).

Hope it's fine just like that!!!
