I was searching CIC website to get the medical form so I can book a date with a Dr. and I came across this link.

http://www.cic.gc.ca/english/information/medical/dcl.asp

At the top it says. "List of countries/territories for which an Immigration Medical Exam (IME) may be required for temporary resident applicants" And I am out of Luxembourg and it shows Paris and says "NO" for "Immigration Medical
Exam (IME) required"

Do I still need it or no? Confusing! :-\

Does anyone know where I can get this sheet? I wanted to send it in with the application. Or do I need it from the Embassy to book it first? I did read and find this part.

Plan your medical examination

If you are still uncertain about whether you need a medical examination, consult a visa office near you.

If you do need a medical examination, your visa office will provide you with instructions and a form with your assigned file number before you see a panel physician.

Your own doctor cannot do the medical examination. You must see a physician on Canada’s list of Panel Physicians.

That would be in the country specific instructions here:
http://www.cic.gc.ca/english/information/applications/fc.asp#region

It is odd however. Your country kit does not indicate anything about a medical examination, only to include the IMM 1017 as proof you did it. (you get this from your doctor).

However,
http://www.cic.gc.ca/dmp-md/medicalinfo.aspx?CountryID=1883&CountryName=Luxembourg

Shows there are no registered doctors in Luxembourg to do this exam. Completely weird, and hope you can figure out what you need to do from other Luxembourg applicants.

That's because my country is very TINY! And not a lot of people go immigrate to Canada from here. They all stay here really. lol.

Yeah I seen those links. But the medical I would get from the Dr. correct? I don't need the Embassy to give me some kind of a number or document to do it? But do I download some kind of a form online and take it to the Dr. so they know what to do or they just already know what it's all about?

I am not too sure what you should do.. Maybe contact CIC (call, don't email as that would take for ever for a reply), and find out the details if you cannot find answers here.

In my country kit, there was a form you print and bring to the doctor, but your kit does not have this.

You will require a medical exam, as all countries require this regardless, so I think they may send you to a neighboring country to get it done.

Again, call CIC for details, or maybe search a little on the forums for other applicants in the same situation.

You no longer need to take the form (Appendix C) with you to the Dr. Most of the country specific checklists/instructions no longer have a references to the medical exam - they have been removed. They have been eliminated as per the recent changes to the Immigration Medical Examination Process (you can find the official document here - http://www.cic.gc.ca/english/resources/manuals/bulletins/2012/ob471.asp). You do not need anything from the embassy either. Make your appointment directly with a panel physician. They will let you know how many passport size photos to bring (and you will need to take your passport and a copy along, too). I have seen many posts where the staff at Drs offices still aren't aware of this change that eliminates the Appendix C and still tell people to bring it along. Or some Dr office staff say you require a file number, but that is for other types of visas where medicals are required after the application is in progress - let them know this is for Spousal Sponsorship and you are required to send the proof of medical (IMM1017 that Dr will give you when he has completed the exam) with the application. I would print a copy of the Bulletin to take with you in case they are not up to date with the changes.

No. That link you used is for Physicians, not applicants. You do not take any forms to the Dr. Only the required number of passport photos and your own passport.

You can do a search on these forums of people recently doing medicals...they will all confirm that you do not take any forms with you.

Okay so I contact one of these in France, and only take passport photos with me and the rest they do for me correct? Can I go book a date with a Dr. then or do I have to wait for the Embassy to ask me to give them one?

If you are applying Outland (which I presume you are) you need to go ahead and book the medical exam, as the IMM1017 (proof of medical exam/receipt) that the Dr will give you, needs to be sent with your application. It is best to do your medical exam as close as possible to sending your application, as the medical is only valid for one year. You may read of others here not doing their medicals until the Visa Office requests them, but that is for Inland applications - those Inland applications can take well in excess of a year to process so it avoids repeats of medicals.

When you contact the Dr to make the appointment, they will tell you how many passport photos to bring. (And always take your passport along.)

Good luck with your application!
