Medical Exam, Appendix D?

Hello,

We are currently going through all the forms we need to apply for a sponsorship of my finace for his Canadian Visa once we get married in March here in the UK. We are about to organise his medical exam but the Immigrant's Guide suggests we need to bring Appendix D with us to the exam, but all we can find is an Appendix C (in the country-specific form 3901), is this the same thing? If not, where is Appendix D and how do we get it, we are getting very confused?

Furthermore, is IMM 1017 EFC - Medical Report Section A something we only get from the Doctor, after the exam? I assume that is the case but confirmation would be great.

Thanks for any light you can shed.

I got this from the CIC website. It looks as though they have changed it from D to C.

- all references “Appendix D: Medical Instructions” As of September 2008, this has been changed to “Appendix C: Medical Instructions”

I'm afraid I can't help you with the IMM 1017 EFC medical section. My husband did his medical a year ago and was given some form to add to our file to prove the medical was done, which we sent. The rest of the medical information was taken care of by the doctor he saw in Northamptonshire.

you have to fill out appendix c and take it to the doctor and he ll give u back the appendix stamped and this latter has to be included in ur file thats all it takes ,

1. fill out Appendix C and take it to the doctor

2. when you're done with the medical, you will get a filled out copy 2 of IMM 1017 EFC from them. It's a little green form which you include with the application.

I'm assuming the DMP you're going to will know more about what they're doing then the one I went to. I had to spend two weeks arguing with them to convince them that they in fact HAVE the IMM 1017 EFC form. Apparently few people marry Canadians in Slovenia and then move there. And when I was done with the exam, I had to tell the doctor to give me a copy of it. So when you go for the exam, I'd recommend taking with you the instructions that say the doctor will give you the form and maybe the document checklist where it's listed as required. Just in case. Better to be overly prepared ;)
The procedure with most other applications is different (for students, skilled workers) - there you first file the application and only then they send you to the medical. That's why my DMP was confused, because normally people bring the IMM 1017 form with them. But that's not the case when sponsiring a spouse.

And btw, if you look at the document checklist, Appendix C is not listed. It's just something you fill out to get the medical exam. The proof of the medical exam is the IMM 1017 EFC form and that's the important one.
