If you're applying Inland currently, you can submit the application for an OWP right away. Just make sure you reference your information (UCI, file number, name, DOB, etc) so CIC can figure it out. There's a chance they won't even if you do it right.
Yes, if you apply to Sydney and you require an interview, you will have to go to Sydney. The good news is few couples from visa exempt countries get interviewed. So it's a gamble - but a small one. I'm American and if you look at our spreadsheet (Ottawa, in my signature. Why our VO is Ottawa, I don't know.) only 1-2 people required an interview. One is gsize, who originally went through Mexico City - long story short, he was transferred to Ottawa to avoid bias at the Mexico VO. (I'm saying he really doesn't count towards our interviews! His interview was at Mexico City VO.) If your application has sufficient proof of a genuine relationship, you shouldn't worry too much. It sounds like you've lived together in Canada, so if you included that proof - well, there isn't much better than living together and having interdependant lives, sharing bank accounts, etc.
Basically, interviews are reserved for when the VO isn't certain the relationship is genuine.
I can't tell you about the extension until I know how you got the OWP.
