Are you a single applicant and paid your fees with the credit card that is on your name? If yes, you need to write the client ID on your fee receipt.

Under "Client date of birth" enter your date of birth in the format dd-mm-yy
For "Client ID" mention the 8 digit number printed at the front of your pr card. Do not write PA number that is on the back of the card. It is PR card number. Client ID is always numeric with format xxxx-xxxx (8 digit) printed at front of PR card.
Under "Address" field enter your address ( I hope the billing address is same as your current residential address?)
Under "Postal Code" enter your postal code associated with the address.
Under "Area Code" mention your telephone area code
Under "Telephone No" mention your telephone number.
