When I received our medical request through email, it only had the medical instructions and the CIIP brochure. The next day, I received our medical forms through email with a .pdf attachment. I got five emails because we were five in our family, each email with the same attachment but with each our names printed on each one. That's what you will be bringing to your medical examination.
Depending on where you'll be having your medical exam, you'll be asked to bring different docs. We went to SLEC last Friday. If you want to go there as well, check their website for their requirements and fees: http://www.slec.ph/#.
