And, they DIDN'T answer the question yet.
Actually, I don't have any other good idea about this.
But, CIC web site said that
IMM 1017: Medical Report: Client Biodata and Summary (not available on our website as it will be GCMS-generated along with medical instructions)
IMM 1017B: Upfront Medical Report: Client Biodata and Summary (not available on our website as panel physicians will have these forms in their office. With the implementation of the new IMM 1017B, Appendix C will no longer be required)

I received IMM 1017 form for me from CIC office and my medical practitioner used IMM 1017B forms for my family members.
So, if the statement in CIC website is true, your medical practitioner should (or may) have IMM 1017B forms in their office for clients.
I would say that you can contact with your DMP again and ask IMM 1017B form or try to contact with other DMPs who have this forms in your region.
I wish you solve this trouble as soon as possible.
