I'd recommend going to CIC website. In there, go on Immigrate -> Apply to Immigrate -> Family Sponsorship -> Sponsor your spouse, partner or dependent children -> Apply. You'll find all forms there.
A few questions to understand complexity of your case: Is this not the first marriage for either of you? Are there any dependent children immigrating too?
As far as work permit goes, your husband won't need to apply for work permit once his application gets approved. He'll be a Permanent Resident, so he doesn't need work permit. Of course, if he wants to come to Canada and work while his application is processing, he will need to apply for work permit separately.
