CJG and Steph can you please help me I want to apply for my gcms notes. Is IMM5744E.pdf and IMM5563E.pdf is all I need? And then i need to pay here https://eservicesak.cic.gc.ca/epay/welcome.do? And then send all this to here Attach and put UCI # in Body >>> ATIP-AIPRP @ cic.gc.ca ( wait for 3-4 weeks )? Is this right or i missed something? Will it show in the notes if they have my ppr and additional notes, rprf fee and medical result in all in cem? Thanks.

Did you pay all your fees at once?

I think Steph posted the procedure for the GCMS notes about a month or so ago. I printed off the receipt, filled it in and mailed it.

Yes, that's all right - the consent form, access to information form, and your receipt, emailed to that address and you have until the 13th of May before they stop accepting e-mail requests so do it now!
Make sure you tick the box electronic files and not a physical copy of your file.
And they will show everything up to the date it's generated. I'm due one in the next few days, hopefully!
 
Actually it is only one of those forms. The web page gets you to fill the actual request out and the only form it asked for was the consent. Still waiting to hear something. Well into the 3rd week now.

My wife is getting the re-med on Tuesday and we were wondering do they still issue a receipt? Like will they give her something to send to CEM or is it all digital now? I was telling her to hit CEM in the afternoon after the med but she said last time she did the med they had her wait 2 weeks to get the receipt (but that was almost 2 years ago now).

Cjg - no i we didnt pay my app fee upfront. I remember you saying in 1 of your post that it actually reached 4-6 weeks before cem got the encoded rprf in the systems? Thanks!

Steph - yes i already told my wife to tick the via email si its a bit faster than the snail mail. Thanks!

Scos - did you also request your gcms notes via the web page? By the way your wife will not get another copy for remed to send to cic. Its just the receipt for the remed, the clinic will send the result.
 
She got a receipt before which was to be included in the app. Does that receipt now go to CEM or is that just for our records?

And I did the online ordering. It will ask all the same questions as the ATIP form, then ask for the location on your hard-drive of the consent form (for ATIP), then it will do the payment processing. And once in the process there is no paging back (not sure what will happen if you do, but they say not to do it). They also had a feedback at the end which I mentioned a couple of somewhat vague questions where the help text wasn't very helpful. Generally it wasn't bad as CIC stuff goes. You will be e-mailed a receipt a few minutes later.

Okay! Thats sounds a bit more easier for my wife to fill :D

About the remed no more receipt or return form to be sent to cic for your app. The clinic will do all the work, all your wife need is to have the remed. They will give your wife just a small receipt for payment, keep it for your record. Thanks!

Edit: i mean the cashier will give your wife just the receipt for payment. Thats it, no more form to be sent to cic or cem.

My wife does her chest x-ray and doctor's interview tomorrow. I remember reading some comments a few months back where some panel physicians have refused to do just the x-ray and interview as they don't make enough money from it. I hoping that will not be the case tomorrow for my wife. I told her that if they refuse it then just do the whole thing and I will then have yet another complaint to lodge with CIC.

I presume i have to print this form off and then sign it and then scan it back onto computer b4 i can send it
