I'm not sure that this is the best place but I'm hoping someone can help.

I've been a PR since january last year, but my wallet went missing in November.

The paperwork to replace the card is straightforward enough, however section H asks for my "Permanent residence card number" which is on the back of the card.

What do I do for this field?
 
Adding a question here, did you mention the police complaint number in Section H. I did report my card is stolen (kind of similar story like above), but, i do not have the complaint number.. Is it mandatory to mention it?

You can call the police and ask for a copy or just the number since your case must have been reported recently. Always best to keep a paper copy of documents at home.

Thanks Mate... just one more question, it hasn’t even been a year since I got my PR. Will I still have to do the biometrics.

Why would you think you have to do biometrics? You are a PR.
