If this is the case, then why they don't fill it themselves and share with other agencies. After all they are sharing info with agencies for security/BG clearance in some other format for all other applicants who are not asked to revert with ISF filled forms.
Anyhow it should be as per their SOPs and we can only guess.

Looks like a simplified version of other forms. Presumably they are sharing that data with another agency (internal or external) and only want to provide that agency with certain data.

LVO cannot fill the form themselves as they need up-to-date information from applicants as certain things would/could have changed. It depends on different Vo's to request for such update.

In this web you can search with immigration summary form, and you will find planty of post saying, this can because of security and background chack , people who have lot of travelling history. But no one is clear why only few people requested such form. Some time they ask to fill only main applicant or spouse or both, in my case LVO ask for both, it seems from the post it will delay application time. But no one is clear about the form. In my email LVO clearly mention write all UK address with post code. But in form it say last 10year addresses, why they are more focus on uk address don't know. I will post this form here later today.
Hope everything is ok in my case.

Thanks for your reply. I had attached a scanned copy of FBI Report, DHL's proof of delivery and the covering letter with both my CSEs. The two CSEs were submitted 45 days apart.

Still no response though.

I hope they are not replying because the file is already complete and they are about to send the PPR.
