My wife applied for Canadian Immigration more than 4 years back at NASC, an immigration consultancy. We lost our hope and just want to get some of our money back. The only document we got from NASC is on the below link. What this means? Is our application applied or only an assessment. If this means application is not fully applied, we have a big chance to get a refund. We plan to approach Consumer Protection here in Dubai.

We got a reply from Central Intake Office that our application is not with them, it is at London Visa Office. The application number we got cannot be checked online.

The photos looks exactly the same with my PER email. However, yours is in a letter while mine, with the same wordings, is in an email. Moreover, mine has the CIC logo and name on the top right of the first page whilst yours does not. BUT your letter is back in 2010 which means there could have been changes in the format since then.
If CIC has replied to you that your file is in LVO, then it means your application has been processed. You should also have an access to the UCI number reflected in the PER to check your application status.
