Locating form for live-in caregiver application URGENT!

Hi im new to the forums
Anyways, I am Australian, currently living in Canada.
I have a WHP work visa.
I have been given a job offer as a live in caregiver.
The employer has completed labour market opinion.
I am about to get my medical tomorrow.
Where do i find/which VISA immigration form do i need to take to my medical ?
Is it IMM 1249 or IMM 1295 or something else?
Very confused.

Imm 1295 and apply to a Canadian Visa Office (Buffalo/Seattle) so you can qualify to apply for Permanent Residency later after working as Live In Caregiver for 24 months in three years. Do you qualify as a live in caregiver?

There are four main requirements you must meet to qualify under the Live-in Caregiver Program:

* You must have successfully completed the equivalent of a Canadian high school education. This requirement will help to ensure that if you apply for permanent residence after two years as a live-in caregiver, you will be able to succeed in the general labour market. Studies indicate that the majority of new jobs in Canada require at least a high school education.
* You must have six months of full-time training in a classroom setting or twelve months of full-time paid employment, including at least six months of continuous employment with one employer in a field or occupation related to the job you are seeking as a live-in caregiver. You may have gained your training or experience in areas such as early childhood education, geriatric care, pediatric nursing or first aid. You may also have completed your training as part of your formal education. This experience must have been obtained within the three years immediately prior to the day on which you submit an application for a work permit.
* You must be able to speak, read and understand either English or French at a level that allows you to function independently in a home setting. For example, you must be able to contact emergency services if required and to understand labels on medication. You will be unsupervised for most of the day and may be required to communicate with someone outside the home. A good knowledge of English or French will also enable you to read and understand your rights and obligations.
* You must have a written employment contract between you and your future employer. The contract defines your job duties, hours of work, salary and benefits. The contract also reinforces your employer's legal responsibilities to you. This requirement helps provide a fair working arrangement between the caregiver and the employer and provides both parties with a clear understanding of what is expected of them.

*So is it correct that i go for my medical AFTER a successful application has been approved, and not BEFORE (as i have booked it for tomorrow not being sure of the procedure?)

*If the order is incorrect, will i have to get a medical examination again, or will the one i have tomorrow have a validation period?

The visa office will send the medical instructions/names and addresses of DMPs.

The reason i ask this is that its not a cheap procedure to have to repeat; and the next examinations will not be for around 3 months where i am living.

It is not a cheap procedure. Oh. You mean the next schedule you can have is not till another three months if you cancelled? The visa office will send you a medical request form with your picture on it. Do you already have an approved LMO?

Just another question;

*I am awaiting documents from Australia : can i send my application in 2 separate lots, or can the DMP hold on to my information so i can send it all at once?

Probably send those that are required on the checklist (imm 5488). Where are you applying? Are you going to apply in person? Might help if you have the documents in Item 8.2 pp12-13

im new in this forum, and i want to clear something , coz im planning to send my complete application
this week or lastweek of september to CE Manila, i 'l just want to ask ur opinion coz i already fill up the application form specially on the portion of work exp from the last 10 years ,
im a little bit confused coz when the time i enrolled as caregiver ,i am a working student at the same time,

caregiver training from 8 am to 1 pm mon - fri , june 2010 to feb 2011
then i also work as a CSR in the mall which is my time is start at 2pm to 10pm or take an halfday sometimes ..
i encode on my position title as a CSR /Working student at the same time,

although i have my complete certs and TOR for Caregiver
im just wondering bka ma"confused " consul sa case ko , do i really need to declare my work exp which is masasagasaan yung time na nag aral ako or they consider the working student since i already finished naman?

So its better to adjust your time from caregiver course than your full/part time job. They dont care what is your schedule from your job. They are after your schedule at school. It should be according to their rules. I read somewhere in other forum got a problem with that. You cant be a part time student.

the original schedule is 8am to 1pm mon to fri so its a minimum 25 hours a week,
the name of my school is Saint Dominic College of Asia ,
but i m planning to input the 8am to 4pm mon to fri, which is sometimes we stayed until 4pm to 5pm tlga
and to satisfy the consul,

and i will not submit the sss contribution ,,
plus the latest coe from the job that i am a fulltime during i take the caregiver course .

my one question is do i really need to submit my all certs of employment and contract
from the last 10 yrs work exp which is not related to caregiver .

its more on food attendant an sales associate ..

ok i will do , i will not submit my cert of employment
although i have my sss contribution from them ..

coz im a regular employee, it happen that im working in the mall
so its a mall time and i can go atleast halfday , thats why i had a time to get the caregiver training last yr to feb 2011..

beside do i really need to submit my cert of employment and contract from my previous employer from the last 10 yrs? or pwede hindi na po.. just include it in the application form.. thank u

Are you applying as a live in caregiver? I don't think you are required to submit cert of employments. You just need to enter the periods of employment with corresponding employers.
