What does a letter from friends/family for proof of relationship have to look like? What needs to be in the letter?
Their name? Address? Place and time met? Signature? Anything else?

yes all of that and also put a reason why they believe that this relationship is genuine would be nice too.
well that is what i had in my letters also if you and your spouse gone out with those friends anywhere would be nice to mentioned too i guess.
Well at least i can see it as enough proof and i am sure that other members will reply and tell you better than i do.

I Also had letters from my Boss and coworkers stating how my husband would pick me up or take me to work. Yes and basically a brief explanation of why your relationship is genuine along with approximate dates of when they met you as a couple.

To make it a formal letter, it should include the writers full name, address, phone number and signature. The CIC will never contact the writer of a letter, but putting in all this info makes it look more legitimate and official since most letters are not notarized/certified.

The basic info needed is the writer's relationship to either sponsor or applicant, length of time writer has know them, how they met the spouse/partner, what they think of the relationship, confirm if spouse/partner are living together etc etc. Each letter can include personal stories and anecdotes, and since this is personal for the writer, each one will be different. As long as it gives testimony the relationship is real, that is what counts.

How should the layout be? How did yours look? I want to do it right so I have a higher chance of it being approved

There is no "right" way to do a letter. They can be hand written (make sure easy to read handwriting) or they can be typed/printed. They should all be personal based on who is writing it.

In fact if you make all the letters look exactly the same, it looks kind of suspicious like you just made them all yourself. As long as they have the basic info mentioned above, and give testimony about the relationship, it really doesn't matter at all how they look or in what format they're in.
 
What if they aren't sure of the exact date? My coworkers met him and i'm not sure of the date even. But even including them I have a dozen people.

please what and what did you submit . Because I got a letter dat they don't think my marriage is genuine that I should prove it within 30 days .
