Pls help, i would like to ask if what medical form ang need namin gamitin sa medical ng Lc2 ko?
kasi sabi sa letter namin medical forms have been emailed to us however if not received please refer to instructions on their website.
wala nga kami nareceive na IMM1017, hindi din available sa website. so am I using appendix C/D? thanks a lot!

just in case, photocopy the letter you received which says medical request has been emailed to you but you have not received it actually and present it, if the need arises, to the DMP.

Guys na received ko na yon MR namin today lang pero nag tataka ako hindi ako nakatanggap ng AOR any way this coming week mag pa medical na kami ng anak ko... tanung lang ilang days or months after ng medical yon next step... like I said hindi ako naka received ng AOR, MR na agad...

Littledipper... kasama na doon yon MR form request included na doon yon info. namin with MR Number.... may attachment yon pinadala nilang email naka PDF files yon...

Congrats Boylawin! At least may MR Ka na kahit walang AOR. Ka long as you have the application file number, I think that's fine

Sir Gicosalamat sir, sir mag kano ba ang babayaran sa MR exam dalawa kami and after nito ilan days or weeks and months bago ang PPR naman at after PPR yon visa naman mabilis na kaya ito... inip na inip na yon anak ko makasama mommy nya hehehe...

Medical fees are based on the age of the dependent. please refer below for the required medical exam and fees ( Basis: St Luke's Extension clinic)

MEDICAL EXAMINATION REQUIREMENTS

0-4 years old Physical Examination
5-10 years old Physical Examination, urinalysis
11-14 years old Physical Examination, urinalysis, chest x-ray (CXR)
15 y/o and above Physical Examination, urinalysis, CXR, HIV, Syphilis

Rates effective August 2, 2012.

Specialty consultations available within the clinic are performed free of charge. Referrals from other clinics will be charged accordingly.

St. Luke's Medical Center Extension Clinic does not accept cheque or credit card payment. Fees must be paid in Philippine Pesos. Other examinations, when required, will be charged accordingly.

hello po. bago lang po. manghihingi lang po sana ng tulong/advice. kc po yung sa pamangkin ko nung sept. 2013 pa nya nasend yung LC2 pero gang ngayon wala pa rin narereceived na aor. san po pwd i-follow up yun? pano po sila mae-email? tsaka ano po ang format? tnx po.
