dear we all are here to seek and help others by our experiences no one is perfect here, so please dont take it serious i was just trying to say that we should follow official instructions and you are right to be on safe end we should provide extra documents but it should be logical if they are saying officially that ONE form is required for main applicant then there is no need to send second.

and if we make it practice on any other issue by providing UN-demanded documents could create any issue or problem for any one. so in my opinion we should just provide what they demand.

"Also Choose the Act Under which you are making the request."

i have carefully read the imm5563 and imm5475 and i agree with cocknbull it would be better option to send two imm5475 forms for wife and husband if they both are included in one application.

Reason

when husband fills imm5475 form he provides all his details, full name and date of birth and he signs so its all ok for him. but if we suppose this form is also consent from wife then where is full name and date of birth of wife on this form. there is a place just for wife signature and there is no space for name and date of birth of wife on single imm5475 so she needs to send her own imm5475 if she is included in this application.

in my humble opinion preferably two imm5475 forms should be sent and it is mentioned on page 1 of IMM5563 that " be sure to include the appropriate consent from each person who has reached the age of maturity" that means wife is supposed to send imm5475.

The Official IMM5475 'Consent to Disclosure' Form

The IMM5475 'Consent to Disclosure' Form is an official Citizenship & Immigration Canada (CIC) form on which you provide your consent for CIC to disclose the information in your CAIPS file to us. CIC will not release your personal information (i.e. your CAIPS file) to us without it as to do so would be a breach of the Canadian Privacy Act.

As your CAIPS file contains not just your own personal information, but also that of your spouse and children if any, we will need additional IMM5475 forms to be completed and signed by any persons aged 18 or over that are included in your immigration application. These forms should be filled out with data relevant to the individuals (e.g. your spouse should put their own name and date of birth) but all forms should have the same file number (i.e. that of the principal applicant).

one should tick access to information act($5) means some one is applying for GCMS to get information by law

one should also tick privacy act (proof of citizenship or permanent residence) means some one has allowed other person to gain inforamtion on his behalf and the id of person who is ordering caips for individual is included in application

CONSENT REQUIRED by all as mentioned in the application (except below 18yrs of age) or application will not be processed.

Even though the case is of a sponsor/partner (unmarried) requesting the GCMS, nevertheless, the consent was mandatory.

First of all CAIPS / GCMS can only be ordered by some one who is present in Canada. If you are out side of Canada you can't order on your own and you need some body who is present in Canada and he / she should order on your behalf.

I didn't get you fully but from what I understood is CAIPS notes get updated after any new information is added in your file. You will have to order CAIPS notes again after some time to view any new information and they won't tell you automatically. Thanks

Email is the fastest medium to send. Ask your friend to send scan of everything. Their email is
