Whenever a form doesn't have enough space, you attach a separate page.

Whenever something doesn't apply, write N/A so they know you didn't just forget to fill it in. However if you are just landing as a PR now, you do not need this form as it says here:

Pls explain this statement.... I thought before 3yrs out of 5yrs. In my case, I will land Aug 2011 for 2wks & exit Canada. I will return Aug 2012 to fulfill my 3yrs committment... meaning I must stay in canada for 3yr straight w/o any vacation outside canada, to be eligible for citizenship?

PR requirements are separate and different from citizenship requirements.
To keep your PR, you must live in Canada at least 730 days in your first 5 years as a PR and in any rolling 5 year period after that. Those 730 days can be in one go or broken up as much as you want. Your PR card is valid for 5 years. If you lose it or it is about to expire and you need to apply for a new one, it is not a problem as long as you have 730 days in Canada in the past 5 years or if you are PR less than 5 years that you still could reach 730 days in Canada before your 5 years are up.
To apply for citizenship, you must have at least 1095 days in Canada as a PR in the previous 4 years before you apply. That is, when you apply, they will look at the last 4 years, if you have 1095 days, you can apply. If you stayed for 3 years straight after landing, you could apply. If you follow your plan and don't really move until August 2012, you can apply for citizenship in August 2015. If you take vacations outside Canada during this time, you delay the time you can apply by as long as your vacation is.
