you do not live in Canada for two out of five years;
you are convicted of a serious crime and told to leave Canada; or

1) Are you talking about passport-sized photos? It's advisable to have them with you. You may need some for paperwork. It's, however, not mandatory

2) You can surely use US bank statement as Proof of Funds. But, not like those general bank statements. It should be on official bank letterpad.

I have a question, while landing we need to submit PR application card form. I have used representative for my application. So do I need to use that form 'use of representative' here again along with PR card application?

No, you don't need to submit PR card application form during landing. You can confirm this with the border officer you encountered.

There is no specific format. Use Form B4/B4A as guidance.

You can either print it out or take photographs. Both are accepted. Just make sure item is clearly visible

You need to look into Residency requirements of citizenship. Currently, it's 1460 Days (4 years) with minimum of 183 days per year

However, new Liberal government have repealed Bill-C24 which will bring residency requirements back to 3 years.
