can someone pls help me with the forms required to get stamped from your home country's doctors to show the child's immunization records

You may perhaps get a blank form of vaccination (immunization) for children below 16 years, which should be signed by a qualified medical professional, from your visa office along with your COPR. If you don't get one, you can use a Record of vaccination (or Vaccination Card ) from your hospital or clinic signed by the medial doctor with his seal and signature. Many of the visa offices does not send such form, but some does.

hello i am new here,
can i ask something about the baby book of my kids with the vaccine that they have when they were just a baby in there baby book there just a date and theres no sign from the health worker that give them a shot and now we couldnt find that health worker because shes not working there anymore in goverment public center,so they advice me to update and take my kids to hospital to take a shot some of the listed vaccine before living to canada so that i can get them a vaccine/immunization certificate.

my point is,is it ok to show there baby book with just only date andd no sign by the government health worker and together with new vaccine certification from hospital???

please help...
