In Q 11, I have two trips to my home country. Do I need to mention this in this section.

2018-06 to 2020-02 --> Working full-time &
From 2018-09 to 2018-11 visited (Home country)


2017-02 to 2018-06 --> Unemployed &
In this From 2017-04 to 2017-06 visited (home country)


Each trip was of 2 months.

My question is do we need to list/ mention trips to home country at Q11 of the form or not.

I personally don't think so... Vacations should not be included in that part...

Do I also need to write her new name. If I write her new name. I don't see any relevant option in the drop down menu to select. Please suggest.

My 5 year eligibility starts from 2015-June.

The address before moving to Canada. Should I start from 2015-June or the address that I started to live at that location.

I mean I was living in that location before the eligibility period. i.e from February 2010.

So the from date should be 2015-06 or 2010-02. Which one is appropriate.

I was also residing on that address from March 2015 but I have started from June 2015. Lets see if someone else will enlighten us on this.

While completing Question 10 below is the table of my addresses if I put the actual dates (Earlier I have provided the same dates for Renewal of MY PR. But at that time day was also visible in the form so the dates were like :
From 2018-01-18
TO 2018-01-31
but in Citizenship forms only month is visible so at few places month is getting repeated where at some places where I moved on last day of month its not.

Please let me know if this is fine and there are no gaps between dates. Moreover the Postal Code for some UAE addresses are not available. What should I mention there?

Is her new name not her married name? If so, choose married name. If she just changed her name completely, just writing her birth name in this table should suffice. They will see her new name from other sections of the application you have filled. This is just my opinion.

The most important part here is that there are no missing days or periods of time. If you started living in that location in 2010-02, I would put 2010-02.

The months look totally fine to me. It is easy to see for instance that your last day in the first location was September 30, 2016 and your first day in the next was October 1st, 2016. And there is no problem with months repeating, that is to be expected.
For the postal codes, if there are none, I would probably use 00000 or write N/A with a preference for the latter. Maybe someone else can pitch in.

In 4a - It is said to write the name exactly. The name typed there is in CAPS. Do I need to write the name in CAPS ?

I don't think it really matters as long as the spelling is the same as on your ID.

No.

Travels should be declared on the physical presence calculator.
If you are unemployed, write unemployed.
