DID I FILL IT CORRECTLY ? 2 JUNE IS MY DATE OF LANDING TO CANADA. WHEN I LANDED HERE I CHANGED 2 ADDRESS HERE DURING MY UNEMPLOYMENT IN THE SAME CITY AND THE SAME WAY IN 2020 DURING UNEMPLOYMENT OF 2 MONTH I CHANGED THE PROVINCE . SI FILLED 2 PROVINCES ADDRESS. DURING UNEMPLOYMENT I STAYED AT HOME SO PUT THE ADDRESS OF HOME.

The eligibility period doesn't start from your pr date. The eligibility period is 5 years back from the date you sign your application. Let's say you will sign your application on 28 Nov 2020, your eligibility period will be 28Nov 2015-28 Nov 2020 regardless of when you became PR.
SO WE HAVE TO PUT OUR HOME COUNTRY ADDRESS TO COMPLETE THE 5 YEARS ?

In your case, you landed in Canada as a PR and that was in 2017, so less than 5 years ago. For other people, the cases are different. For example, I was an international student before becoming PR, so in my 5 year eligibility period, the first 3 years are of international student/temporary status, and then 2 years of PR status. This is because they now count time from your temporary status as well, so they ask for the last 5 years. Also, in some cases, people land 5 years ago and they leave Canada and come back later, so they want to know details of last 5 years to ensure you were in Canada for 3 of 5 years.

In your case, you can just put one entry from the 5 years ago date to the date you entered Canada and just put your home country/address. It won't matter since those days won't count towards your physical presence anyway, but it should be added for completeness.

one more question my wife got 2 month gap between 2 passport. so our reason is just dint bother . but for the immigration purpose what could be the logical reason.

You need 5 years of address history if you were in your home country during your first two years of eligibility period ( 2015-2017), enter your home address.

Actually its just pattern and i will put here 2 more years. just let me know that pattern is ok ?

I have one more question : minor birth certificate has two languages English /Urdu , will it work or need to get a translation ?
i went to a notary public and they told me that there is no need to translate it . so plz any body can share his experience.

question: my kids have health card but there is mailing address printed on the cards not home address . so is that fine ?
