Isa lang po, pero AOM (Advisory on Marriage) na po ang tawag dun. Nakastate naman din dun yung name ng spouse mo so isa lang OK na po

Hello June 2014 Applicants

i am just new to here. June 25 sponsor application sent in and approved August 20. Letter recieved in the mail from Manila VO sent september 16, received on the 25th. This letter was asking for my Passport and 3 additional forms they wanted. So excited that they are asking for my Passport already. Here are the 3 forms they want from me and i have no attachments that came with the letter. My spouse and i have received no emails from them, nothing in our spam folders either.

Fully Completed Appendix A (attached) for you and each of your family members, wether accompanying you to Canada or not. Passport details are not required for any family member who is not accompanying you to Canada.

Personal history from March 2014 to present (see attached form--include jobs held, period of unemployment, periods of study and any other extend use of time, such as time spend travelling in search of a country of refuge, stays in hospital, prisons or other places of confinement, as welll spent at home as a homemaker. PLEASE DO NOT LEAVE GAPS, AND USE MONTH-YEAR FORMAT.)

Complete history of residential addresses from March 2014 to present (PLEASE DO NOT LEAVE GAPS, AND USE MONTH-YEAR FORMAT.) (see attached form)

I have sent them an email on september 25 asking for these attachments and hoping that they email me back with the attachments, no reply yet. i have 45 days to complete these and that would be the end of October.
Any help with this would be appreciated

It looks like somebody from CEM is messing up. AOR2 that looks like they are asking for additional documents but actually no documents are required. Or they are asking for docs but then, not attaching which docs they need. Tsk tsk

I think the form is IMM5669 (Schedule A - Background Declaration) for your personal history and history of addressess.

Anyways, just wait for CEM to reply

Yes they are asking for additional forms and never sent the forms they say are attached. Updating personal history and residential addresses. That appendix a i think is just a form with passport information recorded on it, address and email address. They ask for my passport which makes me happy though . The IMM5669 does sound something like they are asking for, but in the letter they are asking for march 2014 till present. Hopefully they dont take long in replying.

Hi, i think they are asking for two (2) recent photographs of yourself and every family member who will accompany you to Canada.
And then the specification of the photo is on Appendix B.


And yeah, this forum helped me alot too. Im glad i found it while i was putting together our applications.

Still waiting for CEM to reply. Been almost 10 days since i sent them an email requesting the attachment that was supposed to be there. I dont know how long it usually takes for them to respond to their emails. I will send it in if it gets to close to the 45 days they have given me to submit the request for the appendix A
