Too many question all together, first of all congratulations you are the rarest lucky person to get LMO and Job offer..!
Answer to your questions
1) Come join work and then apply for FSW-2 PR, which is FSW based on LMO. It is suggested because the latest update for processing at NDVO(new delhi visa office) is 23 months and if you try to wait that long you might lost that job. Secondly you obtained LMO and not AEO so you have to be working and apply PR at same time
2) Yes Medicals are required once again when you file PR.
3) It doesnt affect your work permit if you file PR at NDVO before landing but technically you wont be able to complete application before coming to canada 8) just give a look to checklist ad latest PR requirements.
