hi ladyR im the sponsor, i just checked the scanned copy of appendix A our consultant emailed us. She had it all ready filled out and put N/A in the space for my pp #, but had my name and date of birth filled out in the spouse column and across the top of the page she wrote "sponsoring spouse/canadian citizen" with an arrow pointing down to my name. We just verified the info of my wife and kids pp#'s than signed and dated it and sent it all to CEM.

I have no idea if my pp# is necessary or not, or if this is the correct way to do it, this is just how our consultant filled it out. Hopefully it didn't cause any problems.

hi LadyR, ako hindi ko na nilagay. nakalagay kasi passport number not needed for non-accompanying members and i would assume na it refers to the entry from Manila to Canada. eh in my case, ako lang mag-isa ang pupunta ng Canada.

We decided to leave the passport info's blank, for me and our son. Since our son is here with me in Canada. He was born here, included prin name nia dun sa app.

We'll be sendin it sa Monday. Wala nman additional docs, Appendix A, pics and PP lng nmn nirequest.

I have a feeling na uulan ng visa before July starts.

Hi LadyR. Ask ko lang the Appendix A na additional doc, pinadalhan ka ba ng form? Which form is that sa application? That's also my requirement and nakalagay "attached" pero wala namang attachment...

sis ako din pinadalhan ng ganyan, at nakalagay din dun attached . balikan mo ulit sis, just incase na namiss mo lang, or you can download the form here www.cic.gc.ca/english/information/applications/index.asp hanapin mo nalang po ung appendix a kung talagang wala sa pinadala sayo. hope this would help

Thanks for your reply. What form did you submit as Appendix A? Yung site kasi leads to Application Forms and Guides. Di ko alam which one to download hehe.

appendix A po, at yung personal history ko since 18 y/o and yung residential addresses ko start ng jan. 2012. yung appendix A dun nakalagay kung may dependants ka, naka table po siya.search nyo po Appendix A . yup na submit ko napo yung pp ko together will all the additional docs nung 18 po.
